In times of stress\, I find myself going back to that magical moment where I was giving a hand massage and manicure to a loving\, giggling child who admired their blue\, pink\, and purple nails as they told me about how much fun they were having as they learn about how to take care of pets\, ride their bike (as they followed the rules\, of course!) and listened to the doctor. In this lecture we will look at various contributing factors and what can be done. \nDr. Beth Haynes\, medical director of the Benjamin Rush Institute\, will be meeting with students of the Stony Brook Medical School campus during lunch to discuss the topic of physician burnout and how it affects the patient-doctor relationship. \nPOST-EVENT SUMMARY: Dr. Haynes provided a very nice overview of physician burnout\, recent trends in physician burnout epidemiology\, and some of the root causes of burnout. The continued intrusion of government\, insurance companies\, EMRs and other third parties have eroded the physician-patient relationship. Not only does this negatively impact our patients\, but it also exacts a toll on physicians. \nBeth then transitioned to summarizing some of the research into the neurobiology of human relationships\, and how these insights help us understand burnout. To activate neurohormonal axes in the brain\, humans need meaningful social contact\, including eye contact\, emotional connection\, and physical contact. Pete and Christi Ghaleb. Direct Primary Care is an increasingly popular practice model which focuses on value-based care by offering patients more comprehensive services for a low monthly fee instead of billing insurance. Like a Netflix subscription for medicine\, DPC offers “all you can treat” primary care that is patient-centered\, and eliminates traditional Fee-For-Service billing (Blockbuster). Often for only $50/month for most adults\, patients receive unlimited office visits\, can call/text/email their provider directly\, and get free after-hours visits. In addition\, patients receive discounts on meds\, labs\, and imaging—commonly saving over 90%. Primary care doesn’t have to be expensive\, and DPC shows why. \nThrough this model\, DPC creates unprecedented value and is able to achieve the quadruple aim of: \n(1) higher quality care\,\n(2) lower cost of care\,\n(3) increased access\, and\n(4) greater patient/physician experience. \nThis model is included in the Affordable Care Act\, and is a hot topic in today’s health policy world because of its positive impact on patient care. \nCome sit back and learn about the future of medicine. Afterward\,Dr. Jason Eberl\, Semler Endowed Professor of Medical Ethics at MUCOM will join us for a discussion about what we saw. \nPlease RSVP to ensure enough food for all. \nPOST-EVENT SUMMARY: Twenty students and faculty joined us for “Star Trek Bioethics.” After we watched an episode of Star Trek: Voyager – Season 7 – “Critical Care\,” Dr. Eberl (MU-COM’s ethics professor) opened a discussion about the bioethical issue pertaining to the episode. The episode dealt with the topic of government controlled healthcare\, particularly how the need for cost effectiveness and utility of each patient to society limited who got care and who didn’t. \nIt was an interesting discussion of why it is important to have a diverse medical market so that everyone can get the healthcare they want. We learned about the dangers of a federally operated healthcare system and the need for less government influence. Marguerite Duane and Matt Haden. \nPOST-EVENT SUMMARY: Forty students and faculty took advantage of this opportunity to explore DPC. Drs. Marguerite Duane and Matt Haden spoke about their direct primary care practice called Modern Mobile Medicine. Their model makes use of the direct primary care model but also incorporates home visits and telemedicine to improve convenience for patients. One of their patients attended the talk with her son and explained how beneficial the practice model was for her. Not only can she access very affordable care around the clock\, but she also is able to have a doctor visit her or her son at home\, minimizing the hassle of visiting a doctor’s office. Modern Mobile Medicine is in its early stages but is expanding rapidly. \n\nHad a great time talking to @Georgetown #medstudents last week. There was a very large audience\, and they were actively engaged. The speakers were excellent and were able to share their experiences thus far in medicine and how their new practice is embodying what drew them to medicine in the first place. We had a DPC speaker in the fall who was terrific\, but these speakers were just as good. They not only explained the huge benefits of DPC\, but they also touched on why the insurance-based and heavily-regulated traditional model does not make economic sense or provide patients with the best options or prices. The event attracted those interested in both DPC and primary care in general\, so I feel that a wide range of opinions on healthcare policy were represented. Drs.
